these are all very different words.
Fan: someone who likes a celebrity/movie/book/type of media/etc. Usually people are fans of a person rather than an object.
Example: That guy has a lot of BTS posters, he must be a big fan! Taylor Swift, I am a big fan of yours, can I have your autograph? I really like the Harry Potter series, I have been JK Rowling's fan for many years.
Mania: a period of extreme excitement associated with mental disorders, especially bipolar disorder. This is not a common word at all. You may have seen it in a book/movie/tv show title?
Example: I had an intense feeling of mania yesterday, but today I am depressed.
Freak: someone who is weird/disturbing/creepy. Usually a negative term but can also be a neutral term for someone who is acting wild/crazy.
Example:
Negative: That guy is a freak, he tried to look at my underwear! The big scar on my face makes me feel like a freak.
Neutral: That girl is a freak on the dance floor.

Also “maniac” could be used like freak if someone is weird/wild/crazed, or fan if someone is obsessed with something
